Agency-directed vs. consumer-directed: two ways to receive services

Agency-directed services mean a licensed home care agency employs the caregiver, handles scheduling and supervision, and sends staff to your home. This is the simpler path for families who don't want to manage employment details.

Consumer-directed services put the family in charge: you recruit, hire, train, and supervise your own attendants, and Medicaid pays them through a fiscal agent. Many families love this model because they can hire people their loved one already knows and trusts, set schedules that fit real life, and build long-term relationships. A services facilitator helps with enrollment and ongoing paperwork.

How to get started: step by step

1. Confirm Medicaid eligibility. Your family member must be enrolled in Virginia Medicaid. If they aren't, apply through Cover Virginia or your local Department of Social Services.
2. Contact your Community Services Board. In our area, that's Prince William County Community Services. Request a DD Waiver screening if you don't have a waiver, or talk to your support coordinator if you do.
3. Ask about personal care through CCC Plus. Call the number on your family member's Medicaid health plan card and request a personal care assessment — especially if you're on a waiver waitlist.
4. Complete the assessment. A nurse or assessor will evaluate how much help your family member needs with daily activities. Be honest about the hard days — the assessment determines authorized hours.
5. Choose your model and provider. Decide between agency-directed and consumer-directed services, then select an agency or services facilitator. Your support coordinator can provide lists of providers serving Prince William County.
6. Build the relationship. The best in-home support grows over time. Share your family member's communication style, sensory needs, and routines with the caregiver from day one.
